Before continuing this form, please click the link below to set up your recurring monthly payment. Add the correct monthly payment amount (see below). 

If you want your payment to come out on a certain day of the month do the following.

- First, add in the amount for your monthly payment.
- Next, change "Give this Monthly" to "Give this Every Two Weeks."
- Choose a day in the current month for your first payment. 
- Finally, change the giving option back to "Give this Monthly" and click Continue.

$45 for Individual
$65 for a Couple

One-Time Prorated Fee
If you are starting in the second or third month of the current quarter, you will be charged a one-time prorated fee for the previous one or two months of the current quarter.

For example, if you started in February, you would be charged a one-time prorated fee of $45 ($65 for couples) to account for the month of January. If you started in March, you would be charged a prorated fee of $90 ($130 for couples) to account for the months of January and February. If you started in January, you would not need to pay a one-time prorated fee because you would be starting in the first month of the quarter.
